Is it possible to clear the journal from the first slavers quest?
*spoilers*
I still have those Lieutenant's advices open.
I've killed him at the end of dialog.
A screenshot look like that:

I think it will switch to 'completed' status once you have finished this part of the game (the fort).

I have finished the fort, killed the slavers, dragons etc. I have even exited through the stairs where I "could hear the birds singing" (one of the toughest fights for me).
I have finished the game and proceeded beyond that (sans the castle assault).

In this case, it's a little bug.
